APPLICATION PROCEDURE
The following steps are required for application and admission
to PEACE LUTHERAN SCHOOL
1. Request application materials.

2. Fill out entire enrollment application form.
NOTE:
Parents or guardians must sign and date the form.
3. The application form may be mailed or faxed to

4. New Student Enrollment
If this is a new student enrollment, the school office will schedule a
pre-enrollment interview with the parents and student in order to discuss
expectations of the family as well as the school.
5. Pre-Enrollment Testing.
Students entering Kindergarten through Grade 8
will need to schedule an appointment to take the readiness test.
6. Notification of Acceptance
A decision on enrollment will be made by the administration.
Once notification of acceptance has occurred, arrangements must be made to pay
the registration as well as other necessary fees.
7. Emergency Information Forms
The necessary emergency information forms must also
be completed and returned to the school office.
8. Immunization Records
A record of immunizations must be completed on the Washington Standard Form
and returned to the school office.
